Register with our practice
We welcome new patients at Woodlawn & Oaklane Medical Centre. In order to start your registration process, please complete both forms below (GMS1 Registration form & New Health Check form). This will give us an outline of your health prior to receiving your medical records from your previous doctor. Once your forms have been received, we ask for 3 working days to process your details. After this, we will be able to book an available appointment with the clinician of your choosing.
Please note, all new patients registering with the Practice are requested provide proof of identity, i.e. a passport and a utility bill with your address.
