Repeat Prescriptions are issued with a counterfoil on the right hand side. You just need to tick the items you need and drop it into the surgery.
Alternatively you may post, email on swlccg.woodlawnandoaklane@nhs.net or submit online (available now by clicking the green prescriptions tab to the right!)
We require at least 72 hours (3 working days) notice in order to process your request.
ALL PRESCRIPTION REQUESTS MUST BE MADE IN WRITING AS WE DO NOT ACCEPT TELEPHONE REQUESTS FOR PRESCRIPTIONS.
If you want us to post the prescription back to you, please supply a stamped addressed envelope. Some local pharmacies collect from us so your prescription can be sent to them direct. If you lose the counterfoil, request forms are available in reception. You will be asked to come in for a check-up at sensible intervals in line with policy. A more detailed leaflet on repeat prescriptions is available at reception.
Batch prescribing enables patients to collect regular prescriptions from a pharmacy of their choice without coming into the surgery. Only patients who are stable on their medication are suitable and your doctor will inform you if this is an option for you.
